In most cases an eye exam by a medical doctor IS covered by your HEALTH insurance! Since we bill your insurance directly, your out-of-pocket expense for an office visit is usually only the amount of your co-pay plus a small refraction fee. Call us today... Insurance/Medicare/Medicare HMO's
We will bill your insurance company directly. You are responsible for the applicable co-payment and/or deductible, and also any non-covered services such as refractions (cost - $25) at the time of your visit.
At the time you schedule your appointment we make every effort to obtain your insurance information in order to verify your benefits. This will cut your wait time and allow us to provide you with an estimate of the charges you will be responsible for at the time of your visit.
Medicaid/CHIP
A referral from your primary care provider may be required for your visit. In addition many procedures now require pre-authorization.
Supplemental Insurance
We will bill your supplemental insurance. However, if your supplemental insurance requires a deductible, you are responsible for any payments we may expect from them. Once we receive payment from your supplemental insurance we will promptly reimburse you for any overpayment.
PCCM Referrals
Medicaid requires most of its patients to select a Primary Care Case Management doctor (PCCM) also know as a primary care physician (PCP). PCPs are typically general practice, family practice or internal medicine doctors. Patients that need care from a specialty doctor must be referred by the PCP to the specialist in writing.
